Accumulation of gas after embolization represents sudden death, which could occur with the use of very small particles or after alcohol embolization. But in fibroid embolization the process is supposed to produce ischemia and cause a more gradual tissue loss. The only difference in your technique that I see could have brought this type of destruction is your use of four occluding coils after embolization. These coils could have accelerated tissue death or alternatively they could have acted like a cork on a champagne bottle by entrapping the gases which could have otherwise seeped retrograde into the blood stream. I am aware that you are not using coils in UAE anymore, but there are still others who are using them and should not.

For differentiation of infection from syndrome, you could have confirmed sterility by performing needle aspiration and culture, but even without
the infection, development of sudden pain 2 weeks after the procedure is alarming and leads to
hystrectomy.
