We had a big hospital wide debate with the vascular surgeons over who should be placing IVC filters. Their main criticism, and one which hurt us the most in our debate, was over insertion site thrombosis from the groin approach. The rate in the literature ranges from 41% to 0%, and most of the conclusions align with the specialty reporting (radiology literature says it's not significant, surgery literature says it is). I sincerely believe that IST is not a significant problem after groin approach IVC filter placement, but after that debate, we now use IJ approach, because it is as safe as the femoral approach and it takes away the surgeons main argument. Silly but true.
