Pelvic congestion is common after pregnancy. Fibroids are known to interfere with conception. Therefore, it is logical that both do not coexist commonly. My rule of thumb is that the woman has to present with typical symptoms in the right context of a few pregnancies. CT/US/MRI may or may not help. Venogram with reflux into the ovarian vein has to be there by definition in order to consider this entity. So it is clinical first and venogram second.
