What do you consider to be "remarkably large"? Is it gestalt, an actual size, or does the definition of large change with the pulmonary reserve of the individual patient? Also, would your decision to do thrombolysis change if you thought the clot might be infected. We had a case not long ago where lysis of a potentially infected clot was done and the patient developed an ARDS type picture. Any experience with lysis of infected clots? By the way, in the case I mentioned surgery was no mfelt to be an option due to the patient's overall poor health and lysis was undertaken as a last ditch effort. It was not undertaken lightly.
