Chances are there was an occult dissection that was not flow limiting
at the time but later opened up. IVUS might have identified it but one
could argue that if the gradient looks good it should not be treated
anyway-after all before stents we got away with that reasonably often.
I have now begun to revisit IVUS, and the new catheters make it
reasonably practical to do so. I think it helps in equivocal situations
and will probably be essential for stent-graft procedures.
