We have had a similar clot which went unrecognized until it resulted in thrombosis of the SVC, Right Jugular, Right Subclavian, and Left Innominate veins. Thrombolysis was successful after 72 hrs.(we used arm and femoral catheters for the infusions). We also used mechanical thrombolysis (Amplatz Thrombectomy device) to debulk resistant clot. There was no underlying stenosis to account for the thrombosis, and the pericatheter clot looked just like this one.

We still do PFSS as needed, but I would not have "launched" this one. We have not done as much with infusions as we could but I don't think the catheter really needs to go unless there is a reason to remove it.
