I would agree with transhepatic alcohol ablation. The risk of injury to the liver from leak along the needle tract is a legitimate concern, but the area of injury would have to be minimal. Since he has no liver dysfunction to start with, I can't imagine how even a large leak would be clinically significant. I would do the procedure under CT guidance and start with a small volume to evaluate its spread pattern. The tumor looks necrotic in the center, but one can't always tell.
