I have only had one patient with FMD that did not respond to PTA. I tried slightly oversizing the balloon with a prolonged inflation (2 min) without decreasing the gradient to my satisfaction (it was >10mmHg systolic). Since it was a long segment, I needed a P204 stent. The lesion responded well with no residual gradient, however one could still see slight "beading" outside the stent. Her hypertension improve significantly and she is down to one med at 6mos follow-up.
