Unfortunate situation. I would be tempted to leave this patient alone. His life expectancy is dismal anyways. If the patient is enthusiastic about treatment alcohol ablation would my preferred modality. I would go transhepatically with US guidance on a fluoro table and monitor the ablation fluoroscopically. How much alcohol? It is a fairly large lesion, 5-10 cc? Good luck.
