Very difficult case when the fistula is directly off of the SMA. In the absence of a covered stent to bridge the hole, temporary balloon occlusion with surgical repair may be the only option. If the fistula is long enough, a large wall-stent on the portal side in the SMV might provide a barrier against which coils or balloons could be packed.
