I have a similar case in a young woman with more posterior and lateral thigh involvement predominantly fed by gluteal vessels. I also used an occlusion balloon through which I placed a microcatheter system. Interestingly, I found that placing the balloon too distally within a feeder did not slow flow, as recruitment by more proximal collaterals promoted actual retrograde flow with the vessel to be occluded. I wrestle with the fear of nervous damage in this distribution.
