1) I don't think PA ango was needed.
2) I have had a very similar case which resulted in SVC syndrome. We used mechanical (Amplatz device)and UK over a 72 hour period to clear out the Jugular, subclavian and the large peri catheter "lugie". The pericatheter thrombus was the last to completely dissolve. There was no clinical evidence for PE (we didn't look too hard either). I have done at least 5 full-blown svc cases and never had a symptomatic PE. It has been interesting to find that lysis often stagnates in these clots and mechanical methods are necessary to clean the vessel out fully, perhaps that's why no emboli.
