One of my partners was doing a renal angioplasty and got into the exact scenario you describe. The vessel dissected into an upper pole branch. This eventually thrombosed despite his efforts to tack up the dissection, lyse the thrombus, etc. The patient lost a portion of the kidney - vascular surgery was not inclined to treat, feeling that the risk/benefit ratio was too high.
This wouldn't have quite as big of a deal if the patient had been referred for renal PTA/stent. She actually had leg claudication, but also just happened to be hypertensive... etc, etc.
