Just a few "questions": How long after the gi bleed and cessation of anticoagulation/IVC filter placement did the upper extremity thrombus develop? If the time was > 10 days, I might have considered thrombolytic therapy (yes, direct cath placement, low dose, etc). SVC filter placement is frought with difficulties but the authors did a very nice job of placement/filter choice/case selection. I have placed SVC filters on rare occasion and still am not sure of the safety, though I am pretty sure of the dangers of recurrent PE.
