I use the brachial approach with a Simon-Nitinol filter whenever possible. It's even easier than the IJV approach (which is my next preferred approach), and is very well tolerated by the patient. I puncture either the median brachial or the cephalic vein (if the cephalic is big enough).
I agree that this patient needs a hypercoaguability evaluation.
