I would be careful about the use of venous outflow occlusion especially if I thought the fistula was high flow. There might be some risk of delayed embolization of the PA thrombus. One thought I had was to place a wallstent across the neck of the PA and then place a microcatheter inside the PA and thrombose it. This I think should be done in conjunction with arterial inflow occlusion. You might also consider thrombin from the arterial side.
