I believe the combination of overwhelming sepsis,biliary obstruction, and malignancy is a lethal mixture in this particular patient and death is inevitable despite the most heroic of efforts in hours to days rather than weeks to months. At times the most difficult decision that we face is when to quit. It would be great to get some followup on this one.
