This looks like a very unfortunate situation which could be end-stage. It would be very difficult to obtain a cholanigogram with all these abscesses. If it is a Klatskin-type tumor, you're dealing with a few isolated systems that do not communicate and draining all of them would be tough. At this point I would try and drain as many of these collections as possible and let her settle down. Later, it could be possible to do a cholangiogram and biliary drainage.
