We have also treated several patients with thrombolysis followed by PTA because of the surgical problems associated with this- the result was short term success but recurrence very quickly. A fairly heroic attempt to open the vein using atherectomy was also only succesful for a short time. The approach we ahve now is thrombolysis, then anticoagulate until surgery.
