I would agree with the conservative approach. I think most of the data is reflects the experience with venous occlusive disease where the risk of ischemia is real. I don't believe anyone has a good handle on the impact of waiting on high-flow priapism, but ischemia should be less of a problem. One of the patients that we reported (perhaps the one that Peter or Ziv did!) had a considerable (many months) trial of conservative therapy, but had acceptable function following embolization. It's always going to be difficult to separate a bad result from waiting versus the result of an original injury, unless its a puncture wound from cavernosal injection.
