Coincidentally, this month's JVIR contains an article by Bob Kerlan describing three such cases and reviewing the literature on the subject. We also published such a case in the Teaching File section of SCVIR Syllabus #6, Thoracic and Visceral Vascular Interventions.
With respect to having this case on the Case Club, it adds a new dimension to the phrase "log on, log off..."
