I do not use Urokinase unless I clearly see a filling defect (which can still be an old clot). Stenting across joints will always be conroversial and raise eyebrows. I placed Wallstents across the knee elbow and shoulder. There was no problem with joint function after that. I could see a problem if you leave the edge of the stent right at the joint. While flexing you could perforate the vessel under those circumstances. I tend not to cross joints in young patients for obvious concerns.
