I remember seeing an identical case during my first post-fellowship year. Not seeing a lot of arches before I got all excited and proudly called the trauma surgeon to report a positive arch. Bill Blaisdell (trauma and vascular surgery guru) looked at the films and with a smile on his face he said: "We'll let this one go..". In his 30+ years of trauma experience he cracked a few chests for these kind of lesions but was not convinced that a repair was required. Since then we had a few of these intimal injuries that we left alone with no adverse consequences to the patients. We should put them all together and write'em up.
