In the 1999 M&M meeting a case of gelfoam pledget placed between two coils in an attempt to stop a GDA bleeder, as a result there was injury to the surrounding tissues. One comment at that time was that even pledgets fragment and can cause injury to small vessels. While I agree that this is not common but, I wonder if it might happen if a smaller catheter such as a tracker was required.
