The part of the graft distal to the fistula does become atrophic (poor flow), and can occlude; I have seen both of these happen

I have never needed a venous balloon to successfully coil one of these; I suspect that the other respondent's problem was due to coil choice. I generally use the old-fashioned .038 Gianturco coils, which if properly sized, will stay where you put them. I have never spent much time doing one of these, have never lost a coil, and have never failed to close a fistula. This is generally really easy to do.
