Are you saying that an AV fistula in the upper or mid graft predisposes to distal graft stenosis? What would be the mechanism of that? I guess one of the theories re. vein graft stenosis is that increase flow may cause some injury that stimulates intimal hyperplasia. An AV fistula should decrease flow in the distal graft and increase flow in the upper graft.
Losing a coil in an AV fistula with a short neck is something that one should be afraid of, especially when there is a lot of flow. One way to prevent this is to go antegrade on the venous side and put a balloon across the fistula while embolizing.
