performing thrombolysis on any septic thrombus is often frought with peril and can disseminate a localized. i agree with dr stein's point regarding debulking of the clot by aspiration and/or catheter drainage to be accompanied by appropriate antibiotics. additionally, if the septic source is not adequately treated ,as this is likely the nidus for thrombus formation, thrombus will most certainly recur even if the initial lytic procedure is successful.
