I agree with the clinical approach to this problem. An embolization procedure may be simple but it adds to the cost quite a bit. The small fistulas sometimes thrombose spontaneously. Our approach is to try and compress it with US on the fluoro table first. You'll be surprized how many (especially the smaller ones) disappear with this simple approach. We do embolizations for the more significant fistulas with clear long neck. This approach works for us.
